Our mission includes breaking down barriers in health policy and delivery to ensure equitable health opportunities. UHF’s emerging new line of work, Health Equity Regional Organization (HERO), works to achieve this vision by bridging and advancing quality and equity and supporting the delivery of health-related social needs (HRSN) screening and services.

    Our work focuses on:

    • Enhancing care for over 7 million Medicaid members in New York.
    • Bridging quality and equity in Medicaid programs.
    • Supporting health-related social needs (HRSN) services to address barriers to care.

    About the Role

We are seeking multiple Program Managers to join a dedicated team working to improve the Medicaid program by reducing health disparities, advancing quality and health equity, and supporting the delivery of HRSN services.

In this dynamic role, you will apply your expertise in health equity, project management, qualitative research, community engagement, and communications to facilitate regional needs assessment and planning activities. You will collaborate with diverse stakeholders, use quantitative and qualitative data to conduct gap analyses, and gather community input to inform regional health equity plans.

    • Work closely with the internal director and regional liaisons to develop a structured project management approach.
    • Execute discrete project deliverables across multiple regions in New York State.

    Research & Needs Assessment:

    • Collect and analyze existing regional health equity and needs assessment resources to identify both community needs and strengths.
    • Collaborate with internal and external partners to develop and publish regional needs assessments and gap analyses.

    Stakeholder Engagement & Communication:

    • Ensure appropriate regional stakeholders have access to the project portal for sharing information, convening meetings, and gathering input.
    • Design and facilitate in-person and virtual stakeholder engagement sessions, including follow-up surveys and data collection.

    Stakeholder Portal Management:

    • Work closely with the director, administrator, and regional liaisons to manage portal content, communications, and action item requests.

    Health Equity Planning:

    • Collaborate with internal and external partners on the development and publication of regional health equity plans.

    Cross-Team Collaboration:

    • Work with internal colleagues specializing in data analysis, value-based payment recommendations, and public communications to support project goals.

    Required Qualifications

    • Health Equity Lens: Strong understanding of health equity principles and demonstrated ability to apply them in work.
    • Project & Stakeholder Management: Proven experience managing complex projects with diverse stakeholders, ensuring timely and budget-conscious completion.
    • Technical Skills: Proficiency in Microsoft Office Suite and ability to quickly learn new project management tools.
    • Communication Skills: Excellent verbal and written communication, interpersonal, and collaboration skills.

    Preferred Qualifications
