Peer Educators are integral to the Treatment Adherence Program and provide specialized services in a professional environment. This Peer will work to encourage engagement in care and support adherence to treatment by calling clients to schedule or re-schedule medical appointments.
4-hour weekly shift ( Anytime between 9am-5pm Monday-Friday)
- Ensure that DNKA calls are made daily to schedule clients for medical appointments.
- Review SCOUT to ensure that clients can receive mail, and voicemail messages and that address information coincides with eCW.
- Identify barriers to care when talking with clients, document and share information with Peers and Case Managers to assist in getting clients to engage in their healthcare (i.e. transportation issues, housing, lack of phone communication, identifying social and economic barriers).
- Support patients in navigating the healthcare system.
- Participate in Peer Huddle, monthly and quarterly meetings and TGIF (optional)
- Must have good interpersonal skills with the ability to relate to diverse groups of people without judgment.
- Document group and individual encounters in SCOUT for Peer meetings on behalf of the Peer team.
- Maintain appropriate records for tracking duplicate mailings to provide efficient use of funds allocated for postage.
- Comply with HIPAA regulations with the usage of eCW and protect patient confidentiality under HIPAA regulations.
- Act as a patient advocate.
- Address patients by their preferred name and pronoun while providing care.
- Must adhere to all confidentiality policies. It is a direct violation of Health Center policy to share names or case facts concerning any client, patient, or volunteer of the Health Center with any other person except for those involved in the care of the patient/client; Any release of confidential information to any other entity shall be performed by authorized personnel only and shall be accompanied by proper written authorization from the patient/client.
