Provide administrative support for a new program that helps individuals of all ages transition from Long-Term Care facilities to community living situations!
POSITION DESCRIPTION:
- Be an onsite presence at assigned skilled nursing facilities to build an interdisciplinary approach to meet the needs of the consumers seeking to discharge.
- Assists consumers in completing and submitting housing applications.
- Gathers documentation needed to assist with public benefit applications
- Supports the team to operate at the top of their role by completing administrative tasks essential to the success of the program
- Completes documentation in electronic records
- Ensures case documentation meets the standards set forth by the Executive Office of Elder
- Affairs for the Community Transition Liaison Program
QUALIFICATIONS:
- High School diploma or GED required, Associates Degree preferred
- At least 1 year of related work experience and/or interest in working with nursing facility residents including older adults or persons with disabilities who are transitioning from an institutional to community setting.
- Applicant must have a reliable car, current driver's license, and safe driving record.
- Proficient in the use of computer systems including Microsoft Word, Excel, Outlook
- Ability to work both independently and as part of a team
