Who are we seeking
We are currently looking for a highly motivated, detail oriented, caring and experienced case manager to be an integral part of our Project Connect Team. A team player who enjoys working in the field to identify and provide services to the most vulnerable people experiencing homelessness. The ideal candidate will be able to work independently and as an integral team member to assist a person experiencing homelessness. The person will have strong communication, problem solving, conflict resolution, and organizational skills. Bridges is looking for a person who is capable of exercising mature and professional clinical judgment.
Responsibilities:
The Housing Sustainment Case Manager (HSCM) is responsible for case management for those living in permanent supportive housing, finding and obtaining housing, plus identifying sources for housing funding, conducting housing-based education, credit counseling and linkage to appropriate related community resources. Services may include, but are not limited to, assisting the individual with gathering documentation, increasing income, accessing appropriate mental and physical health services, and any other services that can assist an individual to improve his/her quality of life. Responsible for implementation and building landlord relationships and conducting home visits. The HSCM is supervised by the Associate Director of Programs Impact.
