    Centro Nueva Creación (Centro) is a community-based organization active in the Fairhill neighborhood of North Philadelphia. Our mission is to promote resiliency in young people through educational enrichment and engagement with the arts and Latino cultures. We run after school and summer out-of-school-time (OST) enrichment programs for young people in our community. Our programs are a mix of literacy, art, homework help, sports, bomba dance and drumming, and other activities.

    We seek qualified applicants to fill the full-time role of Supportive Services Coordinator. This role will report directly to our Executive Director and Program Director.

    Job Summary:

    The Support Services Coordinator (SSC) will work to make our programs more inclusive and help our students access additional services. This position is expected to work closely with our main partner in providing OST programs – the Public Health Management Corporation (PHMC).

    The SSC will work with our staff to enroll students in our OST programs and refer them to outside services. This includes referring our students and their families to relevant supports such as health and wellbeing resources, immunizations, benefits access, food resources, transportation, clothing, and more.

    The SSC will also act as a liaison between our students, their families and our staff to better understand our students’ needs, provide accessibility accommodations where possible, and prevent program disruptions. Overall, this role supports our students’ participation in our OST programs and helps them get the most out of it.

    Main Duties and Responsibilities:

    • Collaborate with the Executive Director and Site Director on developing an inclusive environment for program participants.
    • Participate in OST and related meetings to maintain alignment with contract/grant requirements.
    • Complete intakes, OST enrollments, and provide referrals to program participants.
    • Identify and document the needs of youth participants, as referred by the Site Director, school, and family or guardian.
    • Collaborate with staff to identify and develop reasonable accommodations. Communicate, document, and support these accommodations with other Centro staff.
    • Monitor the implementation of the plan by checking in with the family or guardian and youth monthly.
    • In coordination with the Executive Director and Site Director, identify and coordinate program-level supports including staff training, technical assistance from PHMC, assistive equipment and technologies, and/or other materials to support students successfully.
    • Document plans, case notes, and interventions and have available on site for review and updates as necessary.
    • Assist our students’ families with registering for services and support with external service providers.
    • Inform families of appropriate community resources.
    • Enroll youth into OST school year and summer programs on CitySpan and/or other related systems. Support tracking daily attendance as needed.
    • Participate in training sessions, required meetings, and special events.
    • Complete other relevant duties as assigned.

    Other Responsibilities:

    • Assist with staff coverage during drop-in and group hours, and/or throughout typical day as necessary to adequately staff the center, including group monitoring.
    • Help maintain student files and database information; track enrollment and progress, and report outcomes per guidance by agency and/or funding source.
    • Participate in staff meetings, department meetings and consultations, and supervisory meetings.
    • Participate in staff professional development as necessary and/or required.
    • Complete other relevant tasks as assigned.

    Teamwork and Collaboration:

    • Work in collaboration with program and all other agency staff to facilitate a team environment.
    • Model effective team behaviors.
    • Demonstrate effective communication skills in building relationships with all employees and youth.
    • Cultivate good working relationships with area service providers and support programs to facilitate access to area resources for our students.
    • Substitute for other staff when need arises.
    • Other relevant tasks as assigned.

    Qualifications:

    • Experience preferred working in a school or OST program setting with students, facilitating meetings, and working in groups.
    • Experience preferred with early intervention, early childhood special education, social work, early childhood education, child development, special education, family counseling, family studies, social welfare, psychology or other comparable social sciences.
    • One or more years of experience working with or providing services to children, families, or individuals with disabilities through care coordination or case management.
    • Strong interpersonal and communication skills and the ability to work effectively with a wide range of students in a diverse community.
    • Experience preferred implementing trauma informed activities that support behavioral health or social emotional learning.
    • Experience and ability to de-escalate conflicts using trauma-informed and supportive methods.
    • Ability to gather data, compile information, and prepare reports.
    • Ability to monitor, assess and record student progress against accommodation plan, and adjust plans and services.
    • Knowledge of community health care and vocational services.
    • Ability to plan, implement, and evaluate service and accommodation plans for children.
    • Problem solving skills and ability to multi-task.
    • Bilingual English and Spanish greatly preferred.

    • Vision abilities required by this job include close vision. Often uses a computer, tablet, or other office technology such as a calculator, copy machine, and printer.
    • The employee is often required to effectively communicate and work in community and/or group environments where multiple conversations and simultaneous sound stimuli (music, traffic, etc.) occur.
    • Retrieve and deliver supplies from storage areas to clients and move boxes and materials weighing up to 20 pounds.
    • Often works in outdoor weather conditions, especially during Spring/Summer months.
