Position Summary
The Case Manager Level 2 provides program support and service coordination for people served in our Life Enrichment Day Support Services Program. Working with persons with disabilities, the Case Manager Level 2 provides community integration and inclusion through recreation, skill-building, education, and job exploration, including positive approaches to learning, systematic instruction, behavioral intervention, case management, and self-care needs. This position also provides transportation services for individuals as needed.
The Case Manager Level 2 position is an excellent opportunity if you are passionate about working with persons with various disabilities. This role is a great pathway opportunity for persons aspiring for careers in Social Work, Human Services, and/or Health Care.
Hiring Bonus $1,000 ($500 payout after 6 months of employment and balance $500 after 12-month work anniversary)
