    JOB TITLE: LGBTQIA2+ Case Manager

    REPORTS TO: Shelter Director

    JOB STATUS: Full-Time, 40 hours/ week, Hourly Non-Exempt $24.90

    WORK SCHEDULE: This position is full-time regular and non-exempt and may require working days, evenings, nights, weekends, and alternate work locations. This position typically works 5 shifts Monday–Friday 2pm-10:30 pm including a 30 minute lunch break.

    POSITION SUMMARY: The ROOTS LGBTQIA2+ Case Manager will help to ensure that young adults experiencing homelessness have the services and support needed to achieve self-sufficiency, this position will specialize in working with homeless LGBTQIA2+ guests but will work with all guests in program. In conjunction with the part-time Case Manager, the LGBTQIA2+ Case Manager is responsible for assisting guests with accessing housing, employment, educational assistance, mental health and chemical dependency support, providing referrals to specialized services, and general case management. The ideal candidate is familiar with standard case management concepts, practices, specific needs and resources for LGBTQIA2+ young adults and available resources in Seattle and King County.

    RESPONSIBILITY: The Case Manager is responsible for acting as a point-person and facilitator of a guest’s care plan. They are expected to collaboratively oversee this plan internally through regular communication with their fellow Case Managers, the Shelter Director, and the floor staff team. They are also expected to collaborate with external partners as requested by the client and comply with reporting requirements as part of ROOTS’ grants and contracts, including completing HMIS data, service plans, case notes, intakes and exits, and other required documentation.

    Specific responsibilities include:

    • Maintain a caseload of guests, providing comprehensive case management services, including but not limited to advocacy, assessment, crisis response and intervention, conflict resolution, assistance in problem solving, care coordination with other providers or organizations, and referrals to other services as needed.
    • Build good rapport with guests through engagement during shelter hours, outreach, and individual case management meetings.
    • Develop individualized service and goal plans by assessing guests’ needs, situations, strengths, and support networks and working with the guest to self-identify goals and areas of growth.
    • Develop and maintain relationships with service partners, organizational allies, and other community services and resources.
    • Work closely with and refer guests as necessary to various community resources; including local, state and federal government assistance programs, e.g. legal assistance, food stamps, and healthcare.
    • Evaluate and track improvement in guests’ life circumstances, including housing and employment status, mental health, and substance abuse.
    • Review Daily Shelter Report emails and internal communication regarding shelter guests, provide input to the team as needed regarding a client’s support plan.
    • Periodic reporting to management related to efficacy of services provided, guests’ status metrics and ad Hoc requests.
    • Conduct regular individual support meetings with guests and be present during shelter operations (9 pm – 10 pm or 7 am – 8 am) at least four times per week to provide case management support and outreach.
    • Participate in staff meetings, trainings, and supervisory sessions as scheduled.
    • Provide case management support during weekly drop-in services.
    • Become trained as a Coordinated Entry for All Housing Assessor and complete housing assessments for guests accessing services.
    • Complete the SOAR Training and required training updates.
    • Attend CEA case conferencing meetings to advocate for housing referrals for shelter guests who have completed assessments.
    • Work closely with direct service staff and Shelter Director to develop relevant programming for guest support.
    • Other duties as assigned by the Shelter Director and Executive Director.

    ACCOUNTABILITY: The Case Manager is accountable to and is supervised by the Shelter Director.

    • Demonstrate an understanding of the role oppression and racism play in the perpetuation of inequity, especially as it relates to young people who are not housed.
    • Experience and familiarity with harm reduction and trauma-informed care.
    • Attend required trainings and meetings.
    • Perform duties as workload necessitates while demonstrating flexible and efficient time management and ability to prioritize workload.
    • Maintain a positive and respectful attitude and strive to create a compassionate, safe, and supportive environment in the shelter and program.
    • Communicate proactively with fellow Case Manager, Program Supervisors, the Shelter Director, Executive Director, shelter staff, volunteers, and organizational partners.

    TRAINING REQUIREMENTS

    • Complete required training shifts in shelter program.
    • Attend all required shelter staff trainings and ROOTS volunteer orientation.
    • Participation in ongoing and outside of ROOTS trainings as assigned.
    • Provide proof of CPR, First Aid Certification and Food Handler’s Card within 60 days of hire.
    • Provide proof of completion of full course of COVID-19 vaccination.

    QUALIFICATIONS

    • At least 18 years of age.
    • Minimum of two years of experience in service provision to youth or young adults, preference given to candidates with demonstrated skill among those impacted by homelessness and oppression preferred.
    • Bachelor’s degree in a related field (Social Work, Psychology, etc.) preferred.
    • Case Management and/ or clinical counseling experience working with individuals experiencing homelessness, mental illness, and/ or substance use preferred
    • Must demonstrate ability to work effectively in a volunteer-driven, anti-racist, harm-reduction, equity-oriented service organization.
    • Must have highly developed communication skills, including demonstrated ability to communicate with a wide variety of people, including those who may be experiencing serious mental illness or who may be intoxicated, and effectively manage conflict as needed.
    • Exceptional organizational skills: well-disciplined, reliable, self-starter, able to work under pressure of shifting priorities and deadlines.
    • Solid understanding of issues related to systemic racism, poverty and homelessness.

    The Case Manager will report to the Shelter Director. Hourly rate is $24.90 and includes paid time off, medical and dental insurance, and an Employee Assistance Program.
