    Community Healthcare Network (CHN) is a not-for-profit organization providing more than 65,000 New Yorkers with primary and behavioral healthcare, dental, nutrition, wellness, and needed support services.

    Our network is made up of 14 federally qualified health centers throughout Brooklyn, the Bronx, Queens, and Manhattan, along with a fleet of mobile vans that bring health services to underserved people in need throughout New York City.     WHO YOU ARE:

    The RN Manager will lead quality improvement initiatives, coordinate staff training, and collaborate with interdisciplinary teams to enhance patient outcomes. This position requires a dedicated leader who can effectively manage resources, foster a supportive work environment, and engage with the community to address diverse healthcare needs.

    POSITION SUMMARY:

    The RN Manager is responsible for overseeing the nursing staff and ensuring the delivery of high-quality, patient-centered care. This role involves managing clinical operations in collaboration with the medical director, implementing nursing protocols, and ensuring compliance with healthcare regulations.

    DUTIES AND RESPONSIBILITIES:

    Leadership and Management:

    1. Supervise and support nursing staff, including RNs, LPNs, and MAs.
    2. Conduct regular performance evaluations, providing constructive feedback and coaching to enhance staff development.
    3. Manage staffing schedules to ensure adequate coverage and optimize resource utilization.

    Clinical Oversight:

    1. Ensure adherence to clinical protocols and standards of care to maintain high-quality patient services.
    2. Collaborate with physicians and nurse practitioners to refine and optimize patient care plans.
    3. Assist with patient triage to prioritize care based on patient needs and ensure efficient workflow in a fast-paced environment.
    4. Administer vaccines and other medications as needed, particularly during staff shortages or to maintain optimal patient flow.
    5. Monitor clinical flow to ensure operational efficiency, identifying and addressing any bottlenecks in the process.
    6. Collaborate with nursing staff to address immediate patient care requirements and facilitate seamless operations.
    7. Support the management team in implementing strategies to enhance patient care and departmental efficiency.

    Quality Improvement:

    1. Develop and implement initiatives aimed at improving patient care outcomes and enhancing service quality.
    2. Address and resolve eCW web encounters promptly and effectively.

    Regulatory Compliance:

    1. Ensure compliance with federal, state, and local regulations, as well as FQHC-specific guidelines.
    2. Prepare for and actively participate in audits, TJC surveys and inspections to maintain operational integrity.
    3. Maintain accurate and up-to-date clinical documentation and records to uphold accountability.

    Education and Training:

    1. Collaborate with the education and training department to reinforce protocols and standards that drive high-quality patient outcomes.
    2. Identify training needs to foster a culture of continuous learning and professional development within the nursing team.
    3. Participate in the hiring process to select qualified nursing personnel.
    4. Engage in new hire orientation and onboarding processes to ensure a smooth transition for nursing staff.

    Patient and Family Engagement:

    1. Foster a patient-centered approach to care that actively involves patients and their families in decision-making processes.
    2. Develop and implement strategies to enhance patient education and improve health literacy, empowering patients to take an active role in their care.
    3. Performs other related duties as assigned

    EDUCATION & EXPERIENCE REQUIRED:

    1. Nursing Degree from an accredited institution, BSN preferred
    2. Currently registered to practice as a Registered Nurse in New York State
    3. Current CPR certificate or plan to attend a CPR training course within six (6) months of employment
    4. Three (3) to five (5) years of clinical nursing experience
    5. One (1) to two (2) years in a supervisory or management role preferred
    6. Experience in a primary care or community health setting, preferably within an FQHC
