Immigrant Project Health Worker

Centro Comunitario CEUS (ceusnj.org) is looking to hire an energetic, bilingual (Spanish-English) individual with a passion for public health and a commitment to the immigrant community to join our health team as a Community Health Worker (CHW).

Job Title: Community Health Worker (CHW), Proyecto Buena Salud

Status: 30 hours a week

Term: Feb. 3, 2025- July 31, 2025

Location: North Bergen, NJ

Note: The position is funded for seven months but our aim is to continue the project – and the position - for longer, funds permitting.

Background:

Since February 2021, CEUS has deployed CHWs to work with the Hudson County immigrant community in a project funded by the NJ Department of Health (NJ DOH). The project aims to develop and implement effective health communication and community engagement strategies to reach immigrant communities. After initially focusing on informing immigrants about the COVID-19 vaccine, the project in 2022 broadened and our CHWs now provide information and referrals on a wide range of health services in a program called Proyecto Buena Salud (PBS).

Summary of Principal Responsibilities:

The CHW, either a certified CHW or someone who will be trained by NJ DOH, will conduct outreach in immigrant communities in Hudson County. The CHW will make connections with area immigrants, interview them about their health needs and those of their family, and put them in contact with area health agencies who can provide services to address those needs. The CHW will be responsible for documenting the outreach efforts and recording the substance and outcomes of each case.

The CHW, in collaboration with the supervisor, also will be responsible for researching the availability of different health services in the community, and how immigrants – through the CHWs - can access them.

Essential Functions/Responsibilities:

The key responsibilities of the CHW include the following:

1. 1. Execute outreach plans and strategies to communicate information about the project and health issues to immigrants in the Hudson County area.
   2. Conduct telephone and in-person interviews and informational sessions with immigrant. Determine their needs and make referrals to area health providers.
   3. Attend mandatory 144-hour DOL training (if not already certified).
   4. Look for, and make, partnerships with area health, non-profit, religious and other organizations in the community in contact with immigrant communities.
   5. Help organize and host virtual and in-person community forums on health issues when needed.
   6. Help the supervisor develop and share outreach and information materials with local communities and on social media.
   7. Meet regularly with the Project Supervisor.
   8. Document all interviews/information in project databases to create an ongoing record of each case and provide information for monthly metrics reports required by grantor.
   9. Assist in the preparation of monthly narrative and statistical reports regarding outreach and client counseling efforts to be delivered to the funder.

Minimum Qualifications:

1. 1. High school diploma required; associates or bachelor’s degree preferred.
   2. Public health and/or community organizing experience a plus.
   3. Demonstrated ability to work collaboratively in multicultural contexts.
   4. Fluency in written and oral Spanish, and strong English required.
   5. Ability to attend evening and weekend meetings.
   6. Comfort with public speaking.
   7. Self-motivated; detail oriented, well-organized, and able to prioritize assignments and work load.
   8. Ability to communicate effectively and build mutually respectful relationships with co-workers, partners, and the public.
   9. Familiarity with virtual meeting platforms and with software applications in a Windows environment, including Microsoft Word.
   10. Drivers’ license and access to car preferred; position requires some travel within New Jersey.

Compensation: $20 to $22/hour ( depending on experience); 30 hours/week.
