    About Street Level Health Project:

    Street Level Health Project (SLHP) is an Oakland-based non-profit organization dedicated to improving the health and well-being of underserved, urban, immigrant communities in the Bay Area. Street Level is "a safety net of the safety net" and serves as a point of entry to accessing health and other services for those most often overlooked and neglected, namely the uninsured, underinsured, low-wage workers, and the recently arrived.

    About the Health Access and Wellness Program: SLHP’s health access and wellness integrative model includes patient navigation and advocacy, health enrollment, and prevention services for uninsured and underinsured immigrants and day laborers in Alameda County. These services ensure a holistic health framework that creates access to wellness for people who face significant barriers to attaining resources. It incorporates building strong communities, empowering low-wage workers and uninsured people to access local resources, and advocating for their needs. We also provide weekly food distribution, community space, group health promotion activities, drop-in health, social, and legal referral services, and “warm handoffs” to partner organizations, as well as “know your rights” education.

    Job Responsibilities:

    Program Leadership and Administration

    • The Health and Wellness Program Manager oversees the vision, management, and administration of the health access and wellness and prevention program.
    • Crafting and updating the program vision and strategy in collaboration with the Executive Director, ensuring that the program meets the community's needs.
    • Monitoring and supporting program quality and reporting on deliverables.
    • Responsible for data collection and data entry; works with the Executive Director on grant reporting and provides all necessary data and narratives for grant reports.
    • Tracking program budget and creating reports as needed
    • Engaging in collective organization-wide visioning, strategic planning, programmatic decision-making, and other organizational culture-building activities.
    • Collaborating with partner organizations to advocate for SLHP’s community.
    • Helping with organizational events as needed.
    • Implementing clinical policy and procedures and keep HIPAA integrity with guidance and directives from SLHP Medical Director.
    • Collaborating with outreach staff to ensure that message delivery is accurate, provide new information and messaging as needed, and ensure bilateral communication.
    • Supporting the Executive Director with advocacy efforts at local and state levels.

    Team Management

    • Providing weekly 1:1 supervision and support to staff members.,
    • ensure that the team has the skills, training, support and supervision to function effectively in their roles.
    • Leading regular team meetings that provide an opportunity for your team to gather, collaborate, team build, and problem-solve on challenges facing your program.
    • Conducting annual performance evaluations of team members
    • Conducting work in accordance with SLHP Employee Handbook.

    Health Services Coordination and Patient Advocacy

    • Managing providers and volunteer community health workers;delegate tasks to ensure role clarity and coverage.
    • Ensuring that initial health screenings, 1:1 health education, linkages to care, and navigation operate effectively among health access and prevention services.
    • Ensuring proper patient case management - including follow ups and reviewing patient charts and overseeing the patient discharge process.
    • Maintaining a clean and organized clinic space and restocking supplies.
    • Ensuring patient warm handoff– highlight patient needs and confirm that patients are connected to partner organizations or appropriate medical providers.
    • Ensuring all patient data is entered into the Salesforce
    • Supporting conflict resolution as needed.

    Wellness and Prevention Services

    • Supervising health navigation support, referrals, and wellness services for community members with intersecting barriers to service
    • Supporting health navigators in troubleshooting complex cases, serve as a thought partner, and empower navigators through difficult situations as they arise.
    • Helping create a safe and welcoming environment for community members and staff.
    • Identifying, establishing, and fostering partnerships with counties, community organizations, and other partners to ensure an up-to-date, extensive referral system.
    • Managing benefit (CalFresh and health insurance) partnerships programs.
    • Overseeing and supporting the food distribution program to ensure compliance with food health and safety protocols.

    Volunteer Support & Management

    • As needed, recruiting volunteers to ensure adequate staffing for the program.
    • Recruiting, training, and coordinating volunteer community health workers as needed.
    • Recording hours, managing schedules, and coordinating evaluations for volunteers.
    • Maintaining volunteer program files, training materials, and orientation procedures.

    Required Skills

    • Strong oral and written communication skills.
    • Excellent communication, collaborative, mediation, and group discussion skills.
    • Flexibility, adaptability, and creative problem-solving approach.
    • Experience with Salesforce database management, data analysis, and reporting.
    • Proficiency with Microsoft Office and Google Suite.

    Required Qualifications

    • Bilingual fluency in Spanish and English.
    • Minimum 5 years of experience managing a clinical or health work setting providing care for low-income undocumented immigrants who are trauma-impacted and have experiences of limited literacy and/or substance abuse disorders.
    • Experience with recruitment and outreach in a wide variety of sites, including schools, community events, churches/faith communities, and health systems.
    • Experience in case management, patient advocacy, social services, or peer-based counseling.
    • High level of self-direction, with an ability to multitask and prioritize demands.
    • Knowledge of Alameda County resources, community organizations, and health systems
    • Training in health education, health, nutrition, mental health, or program administration.
    • Interest in policy work that advances the rights of immigrants.
    • Demonstrated commitment to diversity, integrity, and a personal approach that values all individuals and respects differences regarding race, ethnicity, age, gender identity and expression, sexual orientation, religion, ability, socio-economic circumstance, immigration status, and record of arrest or conviction.

    Preferred Qualifications

    • Mam speakers and people of color are highly encouraged to apply.
    • Expertise in Microsoft Excel, SPSS, EMR systems is highly desired.
    • Experience in non-traditional and open community center work settings.
    • Background in Medi-Cal Enrollment and Billing

    Salary and Benefits: This position is full time at a base rate of $72,000- $75,000/year. Street Level Health Project provides full benefits (medical, dental, vision, vacation, paid one-week spring/summer break, paid 10-day winter break, and sick leave).
