Patient Outreach Volunteer

Duties

This position provides outreach to assigned Medicaid patients to schedule them medical appointments at KC CARE. 

· Will utilize spreadsheets to identify patients who need medical appointments at KC CARE.

· Will work with Manager of Care Coordination and Director of Population Health to prioritize individuals who need appointments.

· Will call patients who need medical appointments at KC CARE and ask if they are interested in scheduling an appointment.

· Will schedule patients for appointments following appropriate documentation requirements.

· Will track all outreach attempts to patients on an internal spreadsheet to assist with quality improvement efforts

Qualifications

· Attention to Detail

· Computer literacy

· Must be computer proficient
