    Job Title: Case Manager (CASAC/CTI/Behavioral Specialist)

    Reports to: Director of Social Services

    Summary:

    The Case Manager will work closely with a Social Worker as part of a dedicated team, providing comprehensive case management and counseling services. The role involves assisting individuals in overcoming personal and social challenges related to medical, housing, social, educational, and vocational situations. The Case Manager evaluates clients’ strengths, needs, and goals using assessment tools, records, and interviews to ensure appropriate support services.

    Key Responsibilities:

    • Client Interaction:
      • Conduct client interviews through screening/intake procedures to identify needs related to housing, mental health, substance abuse, employment, and education.
      • Document client histories, assess barriers to self-sufficiency, and define appropriate interventions.
    • Case Management:
      • Provide case management activities, including advocacy and referrals.
      • Follow up consistently to monitor progress and reassess client needs.
      • Maintain accurate and timely records, including progress notes, assessments, and evaluation reports.
    • Program Reporting:
      • Update caseload reports, collect data, and submit activity and program evaluations (e.g., client satisfaction surveys).
    • Client Support:
      • Establish and enforce behavioral guidelines to maintain a safe and supportive environment.
      • Participate in staff meetings and contribute to committees as required.
    • Professional Development:
      • Attend professional meetings, educational conferences, and in-service training to enhance skills.
      • Engage in training related to the Health Realization Approach to improve professional competence.

    Qualifications:

    • Education and Experience:
      • Bachelor’s degree in Social Work, Psychology, Sociology, or related field with at least 2 years of direct service experience, OR
      • Associate degree in Human Services, Business Administration, or related field with 4 years of direct service experience.
    • Certifications:
      • CASAC (Credentialed Alcohol and Substance Abuse Counselor) certification is required.
    • Skills and Competencies:
      • Experience working with special needs populations and knowledge of case management methodologies.
      • Strong organizational and multitasking skills with attention to detail.
      • Ability to prioritize and complete multiple tasks efficiently.
      • Strong interpersonal skills, integrity, and sound judgment.
    • Additional Requirements:
      • Valid NY driver’s license is required.
      • Must be culturally and linguistically competent.
