About the Position:
As Care Manager for Hospice of Santa Barbara, your primary responsibility will be to facilitate the planning, implementation and monitoring of a plan of care for an assigned group of patients and/or families. As a key member of the Patient Care Services team, you will utilize your specialized training to provide compassionate care and support for those facing circumstances related to illness and/or end of life. This position is full-time at 40 hours per week with some remote work possible based on the needs of the agency and patients. Through your meaningful contribution, the Patient Care Services program will continue to blossom, answering the needs of our community.
