Salary & Benefits: $55,000 - $60,000. Schedule: Generally Monday through Friday. Work days and hours may shift depending on scheduling needs; typical schedule will be 10:00 a.m. to 6:00 p.m.; flexibility required.
Summary: The Case Management Coordinator helps address constituent needs by providing constituents with intakes, case management, referrals, and connecting constituents with programming across The Center. The Coordinator is part of the Center Support team, which includes Case Management Services through the Office of Victim Services, Patient Navigation, social support groups, Economic Initiatives and HIV and Sexual Health Services, and will work collaboratively with each team. This role will also work across other programmatic areas, including Mental Health Services and Substance Use Treatment Services.
