Role Description
This is a full-time on-site role for a Community Health Navigator- Benefits at ACLAMO located in Norristown, PA. The role involves day-to-day tasks such as assisting community members in navigating health benefits, providing information on available resources, conducting outreach programs, and advocating for individuals to access necessary healthcare services.
Qualifications
- Strong communication and interpersonal skills
- Knowledge of healthcare benefits and social services
- Experience in community outreach and advocacy
- Ability to work effectively in a multicultural environment
- Fluency in English and Spanish is a plus
- Bachelor's degree in public health, Social Work, Community Health, or related field
Work Details
Full time, hourly. Required to work up to 40 hours per week
Work Schedule
Hybrid remote and on-site work, 4 days onsite 1 day remote. Monday through Friday, 8:30-4:30p. occasional nights and Saturdays.

Special Notes:
- Must have and maintain a valid drivers’ license, automobile insurance and daily access to an automobile and cell phone
- This position requires to work occasional nights and Saturdays
- This position requires visual and auditory acuity and the physical ability to ascend or descend ladders, stairs, ramps, etc. Must be able to adjust, lift, carry, or move objects up to 35 pounds in all directions. Must be able to stand or sit for prolonged periods, walk, and work on your feet for a minimun of 6 hours at a time.
Must obtain the following three clearances:
- Pennsylvania Child Abuse History Clearance.
- Pennsylvania State Police Criminal Record Che
- Federal Bureau of Investigation Criminal Background Check.
