Prisma Health hospice helps patients with a terminal illness live the remainder of life as fully as possible in peace, comfort and dignity – and with less worry. Our kind doctors, nurses and volunteers provide care at home, in the hospital or in one of our nursing facilities.

Skilled nursing

The Prisma Health doctor and hospice team develop a plan of care with the patient and family, focusing on patient goals.

Social work services

Our medical social workers assess the emotional and social needs of the patient and family, provide counseling and assist with community resources.

Chaplaincy services

Our spiritual services are available upon request to people of all faiths and backgrounds.

Home health aide

Certified aides provide personal care services such as bathing, dressing and other daily activities.

Inpatient hospice

Hospice services provided in our licensed facilities or hospitals, on a short-term basis, for pain and symptom management.

Respite care

Services provided in our licensed facilities or hospitals to give family and patient short-term relief.

Continuous care

Extended hospice services provided during a time of intense need for skilled nursing care.

Therapy services

Speech, physical and occupational therapy provided, when appropriate, for easing painful symptoms.
