    ABOUT THE JOB

    The Case Manager Coordinator’s role is to support ICS in the delivery of services of our Supported Living Services (SLS) Case Management Program. The Case Management Program provides one-on-one guidance to individuals with disabilities as they work towards their personal goals and overall independence. Activities can range from teaching cooking skills to a young adult who lives with their parents, to providing conflict resolution between roommates, to motivating a person towards their exercise goals by going on hikes.

    The Case Manager Coordinators role reports to the SLS Director and works collaboratively with the SLS managers and will have supervisory duties with direct service staff. The program requires at least two years of experience with navigating disability services in housing, medical/ mental and behavioral health systems and the coordination of outreach services to partner agencies.

    This job is full time. The typical work day is eight hours, exclusive of the meal period. Staffing needs may require a variation in an employee's starting and quitting times and total hours worked each day of each week. Employees may be required to work overtime or hours other than those normally scheduled whenever necessary. (non-exempt). Salary range: $24-$29/hr DOE.

    DUTIES

    • Write individual service plans
    • Develop and provide assessments
    • Coordinate services and schedules
    • Supervise and train staff including onboarding new staff and ongoing mentorship
    • Provide ongoing training and support to individuals with developmental disabilities
    • Coordinate client annual meetings.
    • In addition to the coordinator responsibilities continue working with clients 1:1 as a Living Skills Trainer.
    • Meet and keep up steady communication with the SLS Director, and other members of the team (including social workers, family, and others) on each assigned case.
    • Provide short term support filling in when other SLS Trainers are not available.
    • Provide positive role model for ICS clients and staff.

    Much of the administrative work will be in matching clients to staff, coordinating schedules, communicating about and coordinating delivery of services, helping staff work together on shared cases, checking over staff paperwork, and troubleshooting problems in the SLS program (e.g. fielding concerns from parents, clients, and staff).

    The coordinator will also work directly to implement plans by providing one on one training with clients, and leading meetings with teams of staff working with client households.

    Due to the nature of our work with our clients, which includes varying numbers and types of referrals, flexibility in fulfilling functions as needed by the agency will be essential.

    This position is excellent preparation for moving into a manager level position.

    QUALIFICATIONS

    • Bachelor’s Degree preferred with at least 2 years of experience working with persons with disabilities, preferably in a residential setting and/or teaching.
    • Experience may be substituted for education on a case-by-case basis.
    • Candidates must possess excellent verbal and written skills, be resourceful, adaptable and able to work within deadlines.
    • In order to meet the needs of the individuals served by this position, a valid driver's license with proof of insurance is required. A background check will be conducted
    • Willingness to be flexible in work schedule assignments.
