    Job Title: Case Manager – Family Supportive Housing

    Location: Boston, MA 02115

    Position Type: Full-time

    Hourly Rate: $24.76 - $26.00

    Overview:

    The Case Manager will provide case management to families with a history of substance use disorders who are exiting homelessness to a permanent housing program. Working collaboratively with their case manager, the family will work to focus on recovery, increase access to resources, improve life and parenting skills, and move toward housing independence.

    Responsibilities:

    • Using Motivational Interviewing and a collaborative approach with the family, the case manager will
    • Upon acceptance to the program, assist client with move-in transition supports including but not limited to furniture and furnishings assistance, budget support, and other resources and services as needed.
    • Within 14 days of move in, conduct an in-depth assessment to best determine the immediate concerns related (but not limited) to financial, legal, mental health, substance use, employment, health care, childcare, education, and housekeeping.
    • Develop an individual service plan (ISP) based on the family’s identified goals, with a focus on recovery and stability.
    • Develop partnerships and collaborative relationships with internal and external service providers to ensure client receives comprehensive services, including DCF, DMH, DPH, and medical and mental health providers.
    • Deliver wrap-around and potentially intensive case management services, with a focus on opioid overdose prevention, nonviolent crisis management and medications for opioid and alcohol disorders, all with a culturally competent lens. All services are expected to be delivered within a harm reduction framework and using a trauma-informed approach.
    • Document all interactions with clients utilizing appropriate reporting mechanisms, including various HMIS systems as well as other systems where appropriate.
    • Monitor units for cleanliness and property damage and address necessary behavior changes with the client as needed.
    • Participates in all required training and professional development activities, including trainings specific to
    • Perform other tasks as requested by the Heading Home leadership team.

    Qualifications:

    • BA Degree with one-year related work experience in homelessness, recovery supports, harm reduction, or trauma informed service delivery. Degree mandate may be waived for applicants with 5+ years of experience specific to families exiting homelessness with a history of substance use.
    • Case management experience with a knowledge and understanding of basic principles of hard reduction and trauma informed care, with a willingness to engage in regular trainings related to providing evidence-based services.
    • Knowledge and experience with service agencies and resources for homeless individuals and/or families.
    • Willingness to accompany clients as needed to appointments.
    • Willingness to work as part of a team to promote the goals of the agency.
    • Willingness to participate in an on-call rotation.
    • Sensitive to the needs of the homeless, low income and diverse populations.
    • Ability to work in a busy, diverse team setting.
    • Excellent time management, organizational and communication skills.
    • Valid MA driver’s license and car required.

    Work Environment :

    Cubicle space within large office building; sitting at a desk and working on a computer, may require equal amounts of sitting and standing, must be able to climb stairs to meet clients in their units. Office is open to the public and visitors only by appointment. Position requires a minimum of four days onsite after training. Office is climate controlled with heat and air conditioning.
