    Description

    Neighborhood Self Help by Older Persons Projects (SHOPP) is a non-profit social services agency located in the Bronx, dedicated to serving minority older adults. SHOPP operates on the philosophy of self-help and mutual assistance, empowering older individuals to address the physical, emotional, social, and financial challenges they face as they age. We believe that older adults are capable of helping themselves and others, improving their ability to manage the complexities of aging.

    NY Connects is a trusted source for free, unbiased information on long-term services and supports in New York State for people of all ages and disabilities. These services include both medical and non-medical support that individuals may need to enhance or maintain their health and independence. The NY Connects program can provide services in various settings, including nursing homes, individual homes, and community-based locations. Our mission is to help individuals live independently while addressing their medical, social, and functional needs that arise from aging or disability.

    NY Connects IA Specialist Responsibilities:

    • Provide Information and Assistance (I&A) to consumers via phone, email, or in person.
    • Ensure compliance with New York State NY Connects Standards while interacting with consumers and performing job duties.
    • Adhere to SHOPP Personnel Policies.
    • Offer unbiased information and assistance on long-term care services and supports, conducting general screenings to assess consumers' needs.
    • Link consumers to appropriate long-term care services and supports, and follow up to ensure successful connections to services.
    • Advocate on behalf of consumers, referring them to relevant programs and offering assistance with applications (e.g., Medicaid, HEAP, Home-Delivered Meals).
    • Acquire and maintain a thorough understanding of local long-term care programs, particularly those relevant to older adults and individuals with disabilities.
    • Maintain accurate and up-to-date consumer I&A records.
    • Perform additional tasks as assigned by the Program Director.
