    Description

    Primo Center is seeking qualified, caring individuals who want to make a difference supporting families experiencing homelessness by working as a Senior Case Manager.

    POSITION RESPONSIBILITIES:

    Provide Intensive Case Management (60%) - at least 60% of the Senior Case Manager’s time will be dedicated to direct client or collateral contacts, specifically:

    • Provide therapeutic case management, community support and counseling services.
    • Develop community partnerships and create MOU agreements with other providers.
    • Support families in obtaining and maintaining mainstream benefits.
    • Provide viable referrals and linkage relating to needs identified on the family/individual service plan.
    • Provide support and resources to adolescents and youth in transition.
    • Provide resources for housing, employment, school enrollment and ways to increase income.
    • Collect and review required program-based intake paperwork.
    • Accurately complete intake paperwork and enter data into the designated data-based systems (HMIS, Client Serve, STARS etc.) in a timely manner.
    • Participate in programmatic meetings and clinical rounds.
    • Facilitate weekly social emotional groups for adults and children.
    • Demonstrate familiarity with Medicaid rules and regulations for behavioral health services.
    • Complete Mental Health Assessments, Service plans, Treatment plans and enter Electronic Records System.
    • Demonstrate knowledge of therapeutic assessments and administer Diagnostic Assessment Tools.
    • Meet productivity requirements of a minimum of 20 direct service hours weekly.
    • Facilitating collaboration and communication between different providers and support services to ensure holistic care for the client.
    • Conducting in-depth assessments of the client's situation and specific needs to identify areas for support and intervention.
    • Working closely with the client to set clear, personalized goals and develop a thorough plan of action to actively address their needs and significantly improve their well-being.
    • Assisting the client in identifying and accessing relevant community resources, such as support groups, housing assistance, and vocational services.
    • Maintaining detailed records of services provided and monitoring the client's progress towards their goals.
    • Providing immediate crisis intervention and support during emergency situations to ensure the client's safety and well-being.
    • Provide intensive case management services for families experiencing homelessness and those at risk of becoming homeless
    • Daily monitoring of family interaction within a communal residential environment, enforce house rules, chores and regulations to ensure a safe and harmonious living space
    • Collect program specific intake paperwork and conduct intakes for all clients (IM+CANS)
    • Support families in securing benefits, such as enrollment in County Care, Medicaid, TANF, SNAP, WIC, Social Security benefits, etc.

    Administration and Client Services Coordination (40%) - a minimum of 20% of the Senior Case Manager’s time will be spent on the following types of meetings/paperwork activities:

    • Maintain ongoing audit ready case files
    • Maintain client engagement and follow-up through a variety of means: phone calls, email, visits, etc.
    • Ensure individuals are supported in the most respectful and professional manner
    • Ensure that health and safety codes are always adhered to and by all residents
    • Attend all scheduled meetings and mandatory training as directed to ensure health, safety, and welfare of the residents
    • Assist residents with daily living skills, conduct life skill training, events and assist program director with projects that support residents and their extended families
    • Support the team in de-escalating crises and stabilizing individuals
    • Stay current with best practices, and knowledge of the field through participation in workshops, training sessions, seminars and conferences
    • Represent Primo Center at events
    • Additional duties as assigned

    Minimum Qualifications:

    • Commitment to Primo Center Mission
    • Valid driver’s license
    • MA or MS degree, preferably in social work, human services or related fields.
    • Ten (10) + years of direct case management experience preferably in social work, human services, or related field.
    • Five (5) + years of work experience, preferably with families experiencing homelessness and/or survivors of domestic violence or human trafficking.
    • Extensive knowledge and relationships with resources and partners.
    • Working knowledge of mainstream benefits and child welfare system.
    • Extensive knowledge of therapeutic assessments.
    • Demonstrated understanding and utilization of Harm Reduction and Trauma Informed Care.
    • Effective verbal and written communication skills
    • Experience and ability to use computers for data entry, communication and report writing.
    • Demonstrate respect for cultural and socioeconomic characteristics of clients and is willing and competent to work with a diverse client population.
    • Ability to work both independently and as a part of a team
    • Must be available for after hours, weekend and on-call work, as needed
