    Summary Statement

    Required: Licensed Social Worker

    The Medical/Housing Case Manager is responsible for assessing, planning, implementing, monitoring and conducting evaluations required to meet clients who are living with HIV/AIDs health, housing and other human service’s needs. Includes developing service plans and coordinating services in a timely, equitable, client- centered delivery of services.

    Essential Duties and Responsibilities

    CFLS uses Apricot software to input, track data and generate reports that provide documentation of our grant deliverable to our funders. Your employment with CFLS requires mandatory compliance from all employees involved in our programs to input information in the pertinent Apricot data fields as a matter of practice. Failure to adhere to this requirement will lead to disciplinary action up to and including termination.

    In performing the duties and responsibilities of this position the Medical/Housing Case Manager will demonstrate adherence to CFLS’ core values through:

    Responsibility

    1. Maintain and follow all facility requirements for Milestone and DBH housing programs
    2. Provide screening to potential participants to determine program eligibility
    3. Conducting comprehensive psychosocial assessments, medical adherence assessments, prepare service plans to address participant needs.
    4. Developing housing plans and follow up.
    5. Coordinate activities and services for the Ryan White Medical Case Management Program
    6. Distribute monthly hygiene, food and other goods allotted to program participants.
    7. Engage in weekly/bi-weekly Case Management session with caseload.
    8. Provide referrals and linkage to other appropriate programs as needed.
    9. Participate in interdisciplinary team meetings both on site and off site.
    10. Maintain good working relationships with property owners and landlords as needed.
    11. Conduct apartment inspections.
    12. Provide intensive and ongoing monitoring of participants to maintain housing stability, provide support and crisis intervention as necessary, and ensure adherence with program guidelines.
    13. Document all client's engagement in a timely manner including goods and services provided through case management, advocacy and supportive services on behalf of clients, home visits accompanying clients to medical and other supportive service visits as required.
    14. Attend and participate in all required training and meetings.
    15. Conduct outreach and recruitment to expand the number of clients served.
    16. Enter and update CareWare database, Apricot, and HMIS.
    17. Participate in monthly supervision sessions with Director of Housing.
    18. Maintain and organize client files.
    19. Other duties as assigned.

    Nature of Work

    Leadership Responsibilities and Reporting relationships

    Has no authority or responsibility for the supervision of staff.

    Oversight and Direction

    • Employee plans and carries out assignments and resolves most conflicts that arise.
    • Receives general direction working from established policies and objectives.

    Minimum Requirements

    Bachelor's degree in social work and 5+ years relevant experience or equivalent combination of education and experience.
