    Heights and Hills, a leading provider of social services to Brooklyn's older adults and their families for over 50 years, has an opening for a Outreach Worker to support our

    Case Management and Friendly Visiting program. This position is full-time.

    Heights and Hills (H&H) provide older adults and their caregivers with the services, support, and information they need to age in place – in their own homes and neighborhoods in Brooklyn. Our mission is to promote successful aging in the community. Founded in 1971, today we are one of the largest providers of social services for older adults in Brooklyn, serving more than 4,000 older adults and their families annually. We provide case management for the homebound, information and support services to families caring for an aging loved one, run a community center for older adults and operate a robust volunteer program.     Role

    The Heights and Hills (H&H) Outreach Worker will be responsible for making connections and cultivating relationships with community partners throughout Brooklyn to broaden the presence of the agency and its programming in the communities we serve. The H&H Outreach Worker will research and identify entities to provide information about H&H Case Management and Volunteer programming and services, attend outreach events to ensure communities are aware of the services available to them. The H&H Outreach Worker will work under the supervision of the Director of Programs but work closely with each Program Director.The H&H Outreach Worker will attend staff meetings, attend training sessions, participate in committees, and work with professional staff of other organizations that provide services to older adults and their families.

    Responsibilities

    • Research and identify community entities to provide information about all H&H Programs
    • Meet with community partner organizations within catchment area
    • Develop and cultivate referral sources
    • Educate community on the supports and services offered by Heights and Hills
    • Attend outreach events
    • Provide community presentations to prospective clients and volunteers
    • Maintain complete and accurate records of contacts and connections as well as

    statistical reports

    • Develop and maintain relationships with other agencies and resources
    • Perform other duties as required

    Required

    • BA or equivalent experience
    • Must be able to work some weekends
    • Have an awareness of how aging impacts individuals and their caregivers
    • Comfort with and/or knowledge of religious organizations in Brooklyn
    • Comfort and ability to work with diverse communities in Brooklyn
    • Skilled in establishing and sustaining interpersonal relationships
    • Ability to communicate with individuals of varying socio-economic backgrounds
    • Public speaking skills
    • Proven ability to work both independently and as part of a team
    • Basic computer literacy – Windows, Microsoft Office
    • Must be able to travel on public transportation to do home visits and community presentations

    Preferred

    • Prefer bi-lingual and cultural competency in Spanish or Haitian Creole.
    • Knowledge of community resources and public benefits eligibility
