Essential Functions
Engage with program participants according to ExitUSA policies and procedures.
Provide individual case management programming, including but not limited to:
- Conducting comprehensive participant onboarding and comprehensive risk assessments for suicide, violence, and other relevant safety concerns.
- Identifying problems, collaboratively establishing goals, and providing interventions designed to meet participant’s needs, including prioritized goals considering the level of need, participant preferences, and desired level of involvement in the case management process.
- Providing appropriate interventions demonstrating knowledge of, and sensitivity toward, cultural diversity, religious, developmental, and educational backgrounds of the population served.
- Providing appropriate crisis intervention.
- Evaluating participant’s progress toward goal achievement, including identification and evaluation of barriers to meeting or complying with case management plan of care, and systematically reassessing for changes in goals and/or functional status.
- Providing education, information, direction, and support related to the goals of participants.
- Providing referrals to appropriate community resources; facilitating access and communication when multiple services are involved; monitoring activities to ensure services are being delivered and meeting the needs of the participant; coordinating services to avoid duplication.
- Facilitates psychoeducational group programming.
- Maintains accurate and timely participant records and participant confidentiality.
- Attends weekly supervision meetings with the Director of Programming and Services.
- Attends program staff and LAH all staff meetings, as needed and required.
- Regular attendance is an essential function of this position.
Other Duties and Responsibilities
- Acts as an engaged and collaborative member of the ExitUSA program team, and LAH teams.
- Attends internal and external conferences, meetings, and trainings, as needed and required.
- Assists the ExitUSA Program Supervisor as requested.
Required Education and Experience
- Master of Social Work degree required.
- At least 2 years of experience working with the criminal justice system
- At least 3 years of experience working with high-risk populations
- At least 2 years of experience with suicide and other risk assessments
- Crisis intervention training is preferred.

- Strong attention to detail and organization skills.
- Excellent communication skills, both verbal and written.
- Computer savvy and high proficiency with a spectrum of technological products and web-based software including Google Suite tools and Zoom; experience with case management software platforms preferred.
- Excellent interpersonal skills, and high comfort level working in diverse environments.
- Ability to work independently and with limited supervision in a virtual work environment.
- Excellent time management and prioritization skills.
- Ability to work cooperatively in a team environment.

Environment and Demands
- Willingness to work flexible hours including early mornings, late evenings, and weekends.
- Must be able to remain in a stationary position for extended periods of time.
- Frequently requires time pressures, emergency situations, and frequent change of tasks.
