    About the organization: Plan A's mobile clinic brings FREE reproductive health care and primary care screenings to rural Southwest Georgia. This listing is for a role focused on Quitman, Randolph Clay, Stewart, Webster, and Marion county and the surrounding areas.

    APPLICANTS MUST BE RESIDENTS OF GEORGIA

    Schedule: This clinic is on the road 3-4 days weekly with the remaining days flexibly worked from our office in Columbus or remote from home. Flexibility to work evenings (maximum 4 times per month) and weekends (1-2 days per month) is required. The schedule is set by the team one month in advance. On days when the clinic is open on the weekend, team members are off the same number of hours during the week.

    Key Duties and Responsibilities:

    60% Health Education and Community Health Worker

    • Communicate with both individual patients and community members in a group setting
    • Perform screenings including blood pressure, blood sugar, STD testing, etc along with counseling on sexual and reproductive health topics.
    • Provide individual patient counseling and education on health-related topics.
    • Onboard patients to our support programs and make referrals to trusted agencies for support with navigating ongoing health and social needs (e.g., housing, transportation, food, violence intervention).
    • Promote healthy behaviors.
    • Collect and track data related to outreach activities, program participation, and health outcomes.
    • Administer point of care/rapid tests and process laboratory tests following proper protocols.
    • Advocate for the health needs of the community and provide referrals to appropriate health services and resources.

    35% Community Outreach and Partnership Collaboration

    • Plan, organize, and facilitate clinic and health education events in the community, at minimum 3 days/week.
    • Conduct community outreach and engage with individuals and organizations to promote awareness of Plan A’s health services and resources.
    • Build and maintain relationships with local community partners, healthcare providers, and community members to support community outreach efforts and increase access to care
    • Provide health education materials and resources to community members on topics such as family planning, preventive care, and chronic disease management.

    5% Administrative and other duties as assigned

    • Participate in team/staff meetings; attend professional development training, workshops, and conferences as needed.
    • Perform other duties as assigned.

    Requirements

    • The candidate must reside in Georgia in or around the Columbus area.
    • The candidate must possess a working knowledge of and established relationships with the local Southwest Georgia community, including familiarity with local history, organizations, agencies, region, and systems. An eye for detail and able to document activities, service plans, and results in an effective manner consistent with the policies and procedures.
    • An independent, self-starter who is also willing and eager to work collaboratively as part of a broader team.
    • Ability to use a computer and other office productivity machinery, such as a label maker, document scanner, and computer printer.
    • The person in this position frequently communicates with patients and/or employees. Must be able to summarize and exchange accurate information in an accessible manner and maintain confidentiality.
    • Ability to work in close quarters with staff and patients aboard a mobile clinic.
    • Ability to walk up and down stairs into the clinic and lift up to 40 lb, assisting in set up and break down of the clinic
    • Driver’s license and access to a vehicle (will be reimbursed for travel).
    • Willing to travel within a 3-hour radius of our Columbus office.
    • Preferred: Experience providing care in a family or women’s health setting
    • Preferred: Previous experience in health education or community outreach
