    Hours: 35 hours per week, Monday through Friday

    Program hours: Monday-Friday, 9 a.m.-5 p.m.

    Salary: Annualized at $54,590.00 with comprehensive benefits package

    Program Description:

    The Geel Webster Avenue Apartments is a single site, supportive housing residence with 116 apartment units consisting of studio, one bedroom and two-bedroom units. Seventy (70) units will house homeless individuals living with serious mental illness (SMI), substance use disorder (SUD), or co-occurring SMI and SUD referred from the New York City Human Resources Administration (HRA). The remaining forty-six (46) units are devoted to Individuals or households who meet the regulatory bodies’ income and household size requirements / admission criteria.

    Supportive case management services are meant to assist individuals to deal with Daily Living Skill concerns as it related to maintaining themselves and their apartments. The major goal areas are rehabilitation, mental health, physical health and socialization and the objectives may include budgeting, medication management, crisis intervention, prevocational training, entitlements, supportive counseling, group dynamics and referral to appropriate service providers.

    Essential Responsibility and Role of a Case Manager

    • Provide case management with supportive counseling and social services
    • Coordinate and provide instructions and information, both individual and group modalities, in the skills of daily living. This will include personal hygiene, maintenance of the apartment, and meeting one’s nutritional needs
    • Coordinate and conduct socialization and recreational activities and groups that foster increased social skills, decrease isolation and improve cohesion and the development of a social network
    • Serve as an advocate assisting tenants in obtaining services that they are required and entitled to. The skills of effectively interfacing with varied social services agencies will be taught. This may involve personally escorting tenants to service provider appointments.
    • Provides availability (on a rotating basis) for crisis intervention and cell phone responsibility during non-scheduled work hours.

    Qualifications and Requirements

    Education and Experience: Bachelor’s Degree or higher. One (1) year’s work experience in the field of social services preferred. Bilingual, Spanish preferred

    Physical Requirements

    • Staff must be able to access offices and apartments above the first floor;
    • Must be able to travel to different locations;
    • Must be able to demonstrate proper cleaning / apartment maintenance methods
