    Position Overview

    The TGIJP Black Trans Cultural Center is seeking a Housing Case Manager to join our team to provide social services navigation and case management services to clients in the TGIJP Housing Program. The Housing Case Manager will focus on assisting the Housing Program team with client related duties as assigned throughout the entire process of locating and obtaining necessary services, employment, permanent supportive housing, and other case management issues as may be identified.

    Some but not all of the specific responsibilities include:

    • Establish and maintain relationships with clients eligible for housing placement through the placement process.
    • Assist clients with the following: program intake documents, internal/external housing program guidance and activities, community resource housing applications, including obtaining appropriate documents necessary for a successful application and successful interviews and in grievance processes should applications be denied.
    • Support clients in moving into new housing should applications be accepted.
    • Assist clients with completing any required processes for housing provider’s certification/recertification processes.
    • Develop and maintain relationships with relevant Property Managers, Leasing Agents, and/or support services staff at the sites where we are referring our clients for housing.
    • Keep accurate, up-to-date records and charts with special attention to client privacy and confidentiality.
    • Provide professional, accessible and culturally competent relevant support, advocacy, and appropriate information and referrals to program participants.
    • Provide crisis intervention management response as needed to clients experiencing acute symptoms associated with their (mental) behavioral health issues; which may be diagnosed or undiagnosed.
    • Prepare weekly program reports detailing activities, client interventions, statistics, and other relevant information that complies with program standards and organizational protocols.
    • Perform other duties as required and/or as assigned.

    Required Qualifications

    • Enthusiasm for TGIJP’s work and a passionate belief in the leadership of Black transgender women within a non-profit organization.
    • Demonstrated awareness of and commitment to a broad range of social, racial, and economic justice issues and an understanding of the multiple communities TGIJP represents (emphasis on currently and formerly incarcerated TGI populations).
    • Substantial or demonstrated knowledge/experience working with individuals in crisis or unstable housing situations.
    • A strong knowledge of issues confronted by and sensitivity to adults with substance abuse, domestic violence, HIV/AIDS, mental health issues, history of trauma and co-occurring diagnoses.
    • Deep knowledge and understanding of the resources available to homeless and low-income residents of San Francisco and surrounding areas.
    • Demonstrates the necessary attitudes, knowledge and skills to deliver culturally competent services and work effectively in cross-cultural situations.
    • Professional and discreet demeanor with good interpersonal skills. Ability to handle confidential information and use of tact, respect, discretion and unbiased judgment.
    • Experience working in integrated service teams using participant-centered, trauma-informed, harm-reduction approaches.
    • Collaborative work style; demonstrated ability to work independently and effectively as an individual and part of a team for successful outcomes.
    • Excellent computer skills and knowledge of Microsoft Office Suite and Google Suite software is required.
