    Description

    Reports to: Street-to-Home Program Manager

    Responsibilities and Duties:

    • Hybrid work primarily in the field, with some remote work from office and home. Conducting street outeach visits and home visits with clients, works in collaboration with other community providers to help clients access housing resources, leasing of units, remain stably housed, promote self-sufficiency, and obtain improved health outcomes.
    • Meet with apartment managers and landlord to ensure stability of client in the residential environment and negotiate new leases to assist clients with maintaining housing stability. Assist clients with completing annual housing certification documents for the government subsidy requirements.
    • Implement Critical Time Intervention (CTI) activities as a time-limited evidence-based practice to support individuals transitioning out of homelessness and into safe permanent housing options. Typical CTI phases are 3-9 months per client case. Total caseload will range from 12-18 at any given time.
    • Work to help clients achieve community integration and continuity of care through facilitating community supports and connections during their initial steps of successful permanent housing residency. This includes homeless populations who are veterans, people with behavioral health challenges, and especially long-term chronic homeless populations.
    • Work intensively in one-on-one relationships with clients to provide support, engagement, and referral through Housing First practices and methodology. Being thoroughly familiar with the practices guiding the work of Bethesda Cares housing-focused street outreach.
    • Maintaining records in HMIS – Homeless Management Information System, including demographics, summary notes, transactions, and assessments in timely manner to comply with contractual requirements.
    • Promotes positive health behaviors to support optimal health and well-being. Refers participants to medical care and mental health services. Ensure clients apply for benefits assistance programs to encourage growth toward independent living.
    • Participate in regular Coordinated Entry Service Meetings/Conference Calls, Attending regular training sessions to enhance best-practice knowledge in the field of homelessness, Housing First, and permanent supportive housing resources.
    • Maintaining and interacting in a culturally sensitive, respectful, and professional demeanor in keeping with Social Work ethics and standards
    • Partner and share insight with Bethesda Cares program staff.
    • Meet with Bethesda Cares program leadership for regular supervision.
    • Must have own transportation and good driving record. Area of service delivery is throughout Montgomery County, MD.
    • Supervision available for Social Work Licensure
    • Other duties as assigned.
