    Position Summary:

    Hijas del Campo is a Contra Costa County-based non-profit organization that aims to help migrant and seasonal farmworkers and their families improve their daily lives, working conditions, health, and safety. We aim to ensure every farmworker has access to the essential resources necessary to thrive.

    The Community Health Worker Coordinator will play a vital role in advancing Hijas del Campo's mission to improve the daily lives, working conditions, health, and safety of farmworkers and their families. This position is responsible for coordinating HDC’s Community Health Worker’s (CHWs) while providing support for community events. Candidate must be forward thinking, and have technical skills to navigate tech components. The ideal candidate is passionate about Campesiné (farmworker/campesinX) rights, culturally competent, and committed to meeting people where they are.

    Key Responsibilities:

    Career Development for Community Health Worker (50%)

    • Lead meetings with up to 10 CommunityHealth Workers (CHWs) with direction from the Health Initiatives Manager.
    • Provide consistent, day-to-day technical oversight for the work of the CHWs, answering questions and supporting them in problem-solving, service delivery, and navigating challenging client situations.
    • With necessary guidance from the Health Initiatives Manager, conduct quarterly one-on-one check-ins with each CHW.
    • Lead workforce development activities such as resume and cover letter workshops, digital literacy training, and job readiness coaching.
    • Support CHWs in identifying and pursuing educational or employment opportunities beyond the program.
    • Identify & support CHW interest list , coordinate onboarding and enrollment into the CHW certification program currently through Aliados Health.

    Direct Healthcare Service Provision (20%)

    • Offer technical oversight for clients’ health insurance enrollment (e.g., Medi-Cal, Covered California) and renewals.
    • Provide direct referrals to local clinics, mobile health clinics and wrap-around services.
    • Educate clients on prevalent health issues in the farmworker community, such as diabetes, hypertension, pesticide exposure, and heat-related illness.
    • Assist with facilitating workshops on workers’ rights, preventative care, and available health resources.
    • Step in to assist with service provision when CHWs are unavailable or require additional support.

    Outreach Coordination (10%)

    • Develop and maintain the weekly outreach schedule across farms, HDC’s headquarters, and community events.
    • Coordinate logistics for field outreach activities, including transportation and materials.
    • Ensure targeted outreach to specific populations, such as H-2A visa workers.
    • Monitor attendance and participation at events and make adjustments to improve community engagement.

    Program Monitoring and Data Management (10%)

    • Train CHWs to accurately log services, referrals, and client interactions in HDC’s case management system.
    • Review data entries for quality and consistency, providing feedback and additional training as needed.
    • Track outreach outcomes and service delivery benchmarks (e.g., insurance enrollments, workshop attendance).
    • Prepare monthly and quarterly internal reports to support program evaluation and funder reporting requirements.

    Community and Partner Engagement (10%)

    • Maintain strong working relationships with community partners, including health clinics, CBOs, local farms/Ag producers, and local Ag contracting agencies.
    • Coordinate with partner organizations to co-host events, deliver services, or share resources.
