    Hours: 35 hours per week, schedules (in person or in the field)

    Program hours: Office is open from 8:00 AM to 7:00 PM Mon. to Fri. and Saturdays 8AM to 4PM

    Salary: Annualized at $55,120.00 with comprehensive benefits package plus sign on bonus of $3000.00

    Program Description:

    The Apartment Treatment Program, Community Residence is a 54 bed, scattered site program. The program is licensed by the NYS Office of Mental Health and designed for single adults diagnosed with a mental illness who will benefit from a shared living arrangement as they work on the skills that will enable them to successfully transition to more independent supportive housing. Residents have their own bedrooms in scattered sites, two or three-bedroom apartments.

    Essential Responsibility and Role of a Visiting Counselor/Case Manager

    The Visiting Counselor/Case manager has two areas of responsibility: program and case management. The Visiting Counselor/Case Manager is responsible for making scheduled apartment visits, delivering supportive services, and making entries in case records of clients with mental health diagnosis who are residents of Geel’s scatter-site apartment treatment program.

    • Case Management for a case load of six (6) clients/residents
      • Assists and demonstrates residents in the acquisition of daily living skills (DLS) that include, but are not limited to:

    personal hygiene/grooming, shopping, food preparation and budgeting.

      • Supervises apartment maintenance chores in cleaning when necessary.
      • Accompanies residents to other service providers, as needed, i.e. Public assistance, Social Security, day programs and medical/psychiatric appointments, etc.
      • Provides availability (on rotating basis) for crisis intervention and cell phone responsibility during non-scheduled work hours.
    • Ability to work with consumers and families, and staff in a caring and respectful manner, and with understanding of and consideration for cultural differences.

    Qualifications and Requirements

      • Education and experience: Baccalaureate Degree in Human Services or higher required
      • Bi-lingual Spanish a plus

    Physical Requirements

      • Must be able to access apartments above first floor with or without elevator access.
      • Must be able travel to different locations using public transportation.
      • Must be able to demonstrate proper cleaning and apartment maintenance methods to clients/ residents in addressing their activities of daily living.
