POSITION SUMMARY
The Social Worker has responsibility for participants enrolled in and referred to ARKC. This position is responsible for psychosocial assessments and reassessments. The Social Worker is a member of the Multidisciplinary Team (MDT) and works with the MDT to develop a comprehensive and collaborative Person-Centered Care Plan.
This is a part-time, 20 hour/week, non-exempt, benefited position. The Center’s hours are Monday through Friday, from 8:30am-5:00pm, but work hours for this position will be Monday (9:30 – 3:00pm), Tuesday (8:30 – 2:00pm), Wednesday (9:30 – 3:00pm) and Friday (9:30 – 3:00pm).
ESSENTIAL JOB FUNCTIONS
- Perform social work responsibilities including evaluating psychosocial needs of incoming candidates and current participants, develop participant plan of care, provide supportive counseling to participants and caregivers.
- Record, date and sign progress notes and assessment findings in each participant’s record for each relevant participant contact. Keep up daily flow sheet per care plan.
- Participate in all scheduled Multi-Disciplinary Team meetings.
- Assists with discharge plans, participants holds, as needed.
- Manage the enrollment process, including screening and interviewing applications and those requesting program information.
- Provide referral to other services if ARKC is not an appropriate setting.
- Responsible for tracking, documenting, and updating participating while they are enrolled at ARKC.
- Ensure Medi-Cal, Veteran’s Administration, and other payor paperwork is completed, submitted, and received.
- Refer potential participants to the assessment team for evaluation.
- Attend meetings, conferences, trainings and other appropriate outreach and education events as requested.
- Partner with Activity Department to run psychoeducational groups as requested.
- Collaborate with family and other community resources to ensure the health and safety of our participants.
- Provide caregivers with resources and support and collaborate with them on their loved ones participation in the program at ARKC.
- Assist with lunchtime food service and with loading and unloading vans, as requested.

MINIMUM QUALIFICATIONS
- Applicant must have one of the following qualifications: Master’s in Social Work (MSW) from accredited university with one year experience in social work in a health care setting. A Master's degree in psychology, gerontology, or counseling from an accredited school and has one year of experience providing social services in one or more of the fields of aging; health, or long-term care services.
- Bachelor’s in Social Work from accredited university with two years’ experience providing social work services in one or more fields of aging, health or long-term care services.
- One year experience with older adults required.
- Bilingual a plus; Mandarin or Spanish highly preferred.
- Driver’s License: CA driver’s license with auto insurance.
PHYSICAL DEMANDS:
- Frequent stand/walk, sit, use a computer; use a telephone, grasp lightly/fine manipulation.
- Regularly required to walk, stand, bend, stoop, twist, kneel, reach, carry
- Regularly required to push wheelchairs.
- Ability to lift, transfer, push/pull, maneuver and reposition 35 lbs.
- TB and health clearance required.
Hours and Compensation
This is a part-time benefitted nonexempt 20 hour per week position. Pay is $35.00 to $37.00 per hour for Master’s Candidates/ $30.00 to $32.00 per hour for Bachelors Candidates; plus - benefits are generous We offer medical, dental, vision, 403B grant (3% at six months), paid vacation, sick leave, holidays, Bronze Avenidas membership, and more.
