DESCRIPTION: This position provides high quality, client-centered intensive supportive services to chronic individuals, families and transitional age youth in permanent supportive housing through Housing for Health. Case management duties range from outreach and engagement; intake and assessment; service planning; housing and rental assistance; on-going monitoring and follow-up; assistance with benefits establishment, transportation, and legal issues; crisis management; eviction prevention; client education; housing location services; and coordination and collaboration with HFH partners. This program includes using harm reduction strategies to enable the participants to permanently retain their rental housing.
REPORTS TO: LA City Manager
SALARY: $25.00 per hour
Duties and Responsibilities:
Ø Outreach, Engagement & Intake
o Process & accept referrals from DHS and other HFH partners per DHS;
o Establish rapport, build trusting relationships with clients and determine appropriateness for program.
o Obtain all appropriate consents & authorizations for clients’ participation in the program and to ensure effective communication with HFH and other service providers providing linkages, resources, etc.
o Refer and link ineligible referrals to appropriate services and linkages;
o Conduct intake and enrollment activities, to include assisting clients with gathering eligibility documents.
Ø Temporary Housing & Rental & Move-In Assistance
o Assist Clients with access to temporary housing, through referrals, until PH placement is secured;
o Assist clients with timely gathering of documentation and completion, submission, and coordination of lease agreements;
o Coordinate client move-in and new tenant orientation to neighbors, property manager, & lease rules;
o Assist clients with accessing services to meet their immediate needs and basic necessities such as food;
o Conduct DHS-approved comprehensive psychosocial re-assessments and update case management services plan a minimum of every three months to include progress toward goals;
o Ensure clients are linked to health, mental health and substance use disorder services by establishing a medical home. DHS/DMH/DPH may also provide services;
o Assist clients with maintaining medication & treatment regimens by accompanying them to appointments;
o Assist clients with obtaining mainstream resource, employment, life skills and ADL’s;
o Assist clients with locating and securing employment, volunteer and/or educational opportunities;
o Assist clients with budgeting and money management;
o Educates clients on tenant rights and responsibilities;
o Works with property managers and HFH to help clients resolve issues that threaten their housing stability and documents all eviction prevention interventions in the client’s file;
o Coordinates transition planning and activities for ICMS clients with moving needs;
o Implement additional services at the County’s discretion based on changes affecting ICMS and/or HFH properties or operations;
Ø Client Satisfaction
o Assess client satisfaction via one-on-one conversations and client/tenant meetings on a continuous basis;
o Administers an annual survey;
Ø Collaboration
o Collaborate with DHS and HFH partners to ensure clients receive the services and support they need to achieve and maintain health, mental health, and housing stability;
o Attend all regularly scheduled meetings with DHS and HFH partners;
o Establish regularly scheduled ICMS case conference meetings to discuss ICMS clients with supervisor and team members;
o Build and maintain collaborative partnerships with community organizations, service providers and public entities to ensure clients have access to a comprehensive array of supportive services;
