    Description

    Job Title: Social Worker

    Location: Gilroy, CA

    Duration:6 Months

    Salary: $35,000 per annum / $33.65 per hour 40 hours per week

    Position Overview:

    We are seeking a dedicated and compassionate Social Worker to join our Health and Wellness Program in Gilroy, CA. This full-time position will involve working closely with a team of six other program staff to provide essential support and resources to clients facing challenges related to behavioral health and substance abuse. The ideal candidate will be a proactive advocate for clients, helping them navigate their needs and access necessary services.

    Work Schedule:

    Monday to Friday, 10:00 AM to 6:00 PM. Some weekend work may be required for program events or community forums.

    Key Responsibilities:

    Client Support:

    • Provide emotional and practical support to clients dealing with behavioral health and substance abuse issues.
    • Conduct assessments to identify client needs and develop tailored action plans.
    • Referrals and Resource

    Management:

    • Facilitate referrals to appropriate behavioral health and substance abuse resources.
    • Maintain a comprehensive database of community resources and services for client access.
    • Case

    Management:

    • Manage a caseload of clients, providing ongoing support and follow-up.
    • Document client progress, treatment plans, and outcomes in compliance with program guidelines.
    • Team

    Collaboration:

    • Work collaboratively with a multidisciplinary team of Program Manager, Program Coordinator, peer support Specialist, Outreach Specialist and Case Manager and other outside healthcare professionals.
    • Attend regular team meetings to discuss client cases, share insights, and develop strategies for effective service delivery.
    • Community

    Outreach:

    • Participate in community education and outreach efforts to raise awareness about health and wellness resources.
    • Build relationships with community organizations to enhance referral networks.

    Qualifications:

    • Bachelor’s degree in Social Work (BSW) or related field; master’s degree preferred.
    • Experience in behavioral health or substance abuse services preferred.
    • Strong understanding of case management principles and community resources.
    • Excellent communication and interpersonal skills.
    • Ability to work independently and as part of a team.
    • Proficiency in Microsoft Office Suite and electronic record-keeping systems.

    Physical Requirements:

    • Ability to lift and transport materials as needed.
    • Comfortable working in various community settings.
