    OVERVIEW

    A Supportive Community For All (SCFA) is a 501(c)3 nonprofit organization established in 2022 with a mission to create a more equitable Snoqualmie Valley by engaging, connecting, and supporting people and organizations to collaboratively build a community where all members can thrive. We draw on collective impact and community power building frameworks while working with community partners to address inequities in health, housing, financial opportunity, and community connection. We practice equity leaderships skills in all areas of our work, acknowledging that the journey to a thriving community is one that creates space to learn, make mistakes, repair, heal, and grow together.

    A Supportive Community For All serves as the “hub” of the Snoqualmie Valley Referral and Navigation Program, a collaborative project of the Snoqualmie Valley Human Services Coalition, that provides centralized access to human services in the Snoqualmie Valley to ensure that community members are connected to the resources they need. We also participate in Healthier Here’s county-wide Community Hub. Case managers play a crucial role in the success of these programs by conducting community outreach, completing assessments, providing resource navigation, and making referrals to meet community members’ needs, particularly related to food insecurity, housing, and/or financial insecurity.

    ESSENTIAL DUTIES AND RESPONSIBILITIES

    • Conduct community outreach to raise awareness and engage community members who would benefit from resource navigation and case management support, both online and in person, often in nontraditional areas such as food pantries, libraries, subsidized housing, or through other community connections
    • Complete initial assessments with community members to better understand their strengths, challenges, and needs
    • Support community members in goal setting, following up at regular intervals to support their progress in meeting goals
    • Make referrals to community-based organizations to ensure that community members are connected to services to meet their needs, utilizing cloud-based referral platforms when appropriate
    • Assist community members with applications for benefits and resources as needed
    • Maintain accurate, up-to-date and complete records of all client and collateral contacts in case management platforms
    • Develop strong relationships with local service providers that will nurture connections and support cross-sector collaboration
    • Represent A Supportive Community For All programs to the community, referral sources, and other parties, when appropriate
    • Participate on community-based task forces, collaborations, or committees that support program success, raise awareness, and encourage utilization of A Supportive Community For All resources
    • Other tasks as requested

    QUALIFICATIONS

    Ideal candidates will have the following qualifications:

    • Commitment to values of SCFA – equity, collaboration, connection, and innovation
    • Understanding of and commitment to meeting the needs of all people living in the Snoqualmie Valley
    • Demonstrated work experience providing care coordination or case management services in a healthcare or social service setting
    • Ability to establish and maintain rapport, and communicate effectively with people regardless of age, gender, or social, economic or cultural background
    • Comfortable with adjusting to change and creating new systems in an uncertain environment
    • Aptitude for solving problems with creativity and resourcefulness
    • Experience with implementing new programs
    • Knowledge of local community resources
    • Ability to respond effectively in crisis or emergency situations
    • Excellent oral and written communication skills
    • High level skills as a computer user, including case management software, databases, Microsoft Office and other Windows applications
    • Bilingual abilities in Spanish or other languages are a plus

    PHYSICAL DEMANDS

    • Office environment – no specific or unusual demands
    • Frequent driving in the Snoqualmie Valley and occasionally the greater Seattle area

    OTHER REQUIREMENTS

    • Must possess reliable transportation and a valid driver’s license
    • Must provide current proof of insurance on personal vehicle at each renewal date
    • Available to attend meetings, functions, and events after normal business hours on occasion
