    Description

Job Title: Community Health Worker (CHW), Proyecto Buena Sal

    Status: 37.5 hours a week

    Term: November 1, 2024- July 31, 2025

    Supervisor: Supervisor, Proyecto Buena Salud

    Location:North Bergen, NJ

Note: The position is funded for eight months but our aim is to continue the project – and the position - for longer, funds permitting.

    Background:

Since February 2021, CEUS has deployed Community Health Workers (CHW) to work with the Hudson County immigrant community. The project, with funding from the NJ Department of Health (NJ DOH), aims to develop and implement effective health communication and community engagement strategies to reach immigrant communities. Now called Proyecto Buena Salud (PBS), the project initially focused on CHWs trained by NJ DOH who provided COVID-19 vaccine information. In 2022, we broadened the project so that our CHWs now provide information and referrals on a wide range of health services.

    Summary of Principal Responsibilities:

The CHW, who will be trained by NJDOH, will conduct outreach in immigrant communities in Hudson County, with a particular focus on the Union City-West New York-North Bergen and Jersey City area. The CHW will make connections with area immigrants, interview them about their health needs and those of their family, and put them in contact with area health agencies who can provide services to address those needs. The CHW will be responsible for documenting the outreach efforts and recording the substance and outcomes of each case.

The CHW, in collaboration with the supervisor, also will be responsible for researching the availability of different health services in the community, and how immigrants can access them.

This is a good job for someone with a strong interest in immigrants, a passion for community work and a desire to work in the health field. It requires a well-organized individual with strong communication and people skills.

The key responsibilities of the Community Health Worker include the following:

    • Assist in executing outreach plans and strategies to communicate information about the project and health issues to other immigrants in the Hudson County area.
    • Conduct telephonic and in person one-on-one interviews and informational sessions with immigrant community members. Determine their needs and make referrals to area health providers.
    • Attend mandatory 144-hour training conducted by the NJ Department of Health.
    • Make partnerships with area health, non-profit, religious and other organizations in the community in contact with immigrant communities.
    • Help organize and host virtual and in-person community forums on health issues when needed.
    • Help the supervisor develop and share outreach and information materials with local communities and on social media.
    • Meet regularly with the Project Supervisor.
    • Document all interviews/information sharing contacts in project databases to create an ongoing record of each case and provide detailed information for monthly metrics reports required by grantor.
    • Assist in the preparation of monthly narrative and statistical reports regarding outreach and client counseling efforts to be delivered to the funder.

    Minimum Qualifications:

    • High school diploma required; associates or bachelor’s degree preferred.
    • Public health and/or community organizing experience a plus.
    • Demonstrated ability to work collaboratively in multicultural contexts.
    • Fluency in written and oral English and Spanish required.
    • Ability to attend evening and weekend meetings.
    • Comfort with public speaking.
    • Self-motivated; detail oriented, well-organized, and able to prioritize assignments and work load.
    • Ability to communicate effectively and build mutually respectful relationships with co-workers, partners, and the public.
    • Familiarity with virtual meeting platforms and with software applications in a Windows environment, including Microsoft Word.
    • Drivers’ license and access to car preferred; position requires some travel within New Jersey.

    Compensation:$20/hour, depending on experience; 37.5 hours/week.

Interested candidates should submit cover letter, resume and two references via email to Project
