POSITION TYPE: Housing Case Manager – Full Time Non-Exempt
DESCRIPTION: This position works with the City of Burbank on the Permanent Supportive Housing Voucher Program to provide assistance with obtaining housing and supportive services on a long-term basis for homeless persons who meet the HUD definition of Chronically Homeless. Case management duties include budgeting and counseling on basic life skills, and advocacy for health care, legal services and benefits, such as SSI/SSDI, education and financial literacy education, and connection to other services as needed. This program includes using harm reduction strategies to enable the participants to permanently retain their rental housing. This position is also responsible for working with homeless families and individuals to develop a personalized case management plan that addresses their barriers to returning to housing. This includes short- and long-term planning for housing and employment search, as well as the above-stated services.
REPORTS TO: CES Supervisor SALARY: $23.00 per hour
Ø Provide supportive service to keep clients permanently housed.
Ø Work directly with the City of Burbank Housing Authority for new client matches;
Ø Coordinate with the individuals and the city of Burbank to verify applicant’s qualification for Burbank PSH, collect application materials, and submit for housing;
Ø Housing Counseling o Work with clients to develop individualized housing plans;
o Adheres to strict boundaries and professional ethics in the care of others;
o Assists clients with housing applications, completes supportive and subsidized housing paperwork, surveys rental market for affordable housing, and advocates for clients with prospective Property Owners.

Ø Coordinate Moves to Permanent Housing
Ø Maintain Client Records o Maintain accurate documentation of service objectives and outcomes as well as other services in accordance with Federal, State, County and Ascencia guidelines;
o Maintain client related data systems, including case notes and complete HMIS entries; o Prepare case management related reports including but not limited to: outcomes, successes, etc;
Ø Home Visits/Client Follow-up o Continually monitor and evaluate each client’s progression through their housing case management plan, and develop corrective action revisions to the plan as needed;
o Meet clients at minimum twice/month to provide strengths-based case management and service coordination services designed to assist tenants in obtaining and maintaining stable housing.
Ø Property Owner/Tenant Relations and Recruitment o Attend program-related community, coalition and committee meetings as assigned; o Conducts crisis intervention as needed.
Ø Case Conferencing o Ensure effective service delivery by coordinating work with other case managers assigned to client.
