    Summary:

    The Director of Case Management will be responsible for designing case management services at CHiPS, both in the residence and food services departments. They will provide counseling and case management for women residing in the Frances Residence and oversee efforts to provide case management services to food services guests. They will meet with up to 9 residents each week to help them set and accomplish goals, oversee efforts to identify resources for both the residents and food services guests, and meet with or oversee volunteers and interns meeting with food services guests to link them with other needed services. This role reports to the Executive Director and works cross collaboratively with all levels of staff, volunteers, and donors at CHiPS.

    Duties:

    • Help design case management services at CHiPS, ensuring support for both Frances Residence residents and food services guests
    • Participate in CHiPS senior management team
    • Oversee or directly provide counseling and case management services to Frances residents and food services guests
    • Identify other services that might be of interest to residents/food services guests via internet research or other means, reach out to develop referral relationships with appropriate organizations, and help maintain relationships, once established
    • Help develop an online system for keeping updated information about referral resources and track the residents/guests who are referred to them
    • Represent CHiPS at meetings with other providers to strengthen relationships
    • Interface regularly with the Director of Residential Services, the Director of Food Services, and the Director of Volunteers and Community Engagement to coordinate services
    • Performs other duties as assigned by the Executive Director

    For the residents:

    • Schedule weekly, 30-45 minute case management sessions with up to 9 residents each week, at times convenient for them, preferably in person at CHiPS, but virtually if needed
    • Conduct intake and assessments for potential residents in collaboration with the Director of Residential Services
    • Assist existing residents to clarify their goals related to employment, education, child care, parenting and permanent housing, and once goals are established, review and give feedback on progress
    • Help the residents identify any additional services they need and assist in accessing them
    • Provide advocacy and support by accompanying residents to appointments with HRA or other city agencies and organizations to assist with and model self-advocacy
    • Support residents in developing independent living skills, including conducting periodic unit inspections focused on habit-building and maintenance
    • Complete case notes in the electronic record keeping system
    • Develop and enforce a discharge plan for residents in collaboration with the Director of Residential Services
    • If needed, oversee social work internship program, including onboarding of, training of, and weekly meetings with, social work interns to help them support trauma-informed programming at CHiPS, which focuses on care informed by the complete picture of residents’ lives and experiences

    For Food Services Guests:

    • Design and oversee a basic case management system to connect food service guests with services provided by partner organizations, and to track referrals including success rates and other important parameters
    • Develop and train a cadre of volunteers and/or interns to interface with guests and learn their needs.
    • Meet with guests or oversee staff/volunteers meeting with guests to facilitate linking them to other needed services

    Qualifications

    • Master’s degree in social work
    • License as a clinical social worker (CSW) or masters of social work (MSW)
    • Proficient in Microsoft suite, Google Suite, Adobe Acrobat web-based meeting platforms and Zoom
    • At least three years relevant counseling experience and two supervising or overseeing a program (total of 5 years)
    • Ability to conceptualize a program, supervise staff/volunteers to implement a program, and form relationships with other social service organizations
    • Excellent engagement skills and ability to relate to and gain the trust of the women and guests and ability to provide growth oriented supervision to social work interns and volunteers providing counseling and case management
    • Excellent verbal and written communication skills, self-motivated, and ability to work as part of a team
    • Training in at least one relevant evidence-based intervention, such as Motivational Interviewing
