    Shift Schedule: Monday-Friday: 9AM-5PMHours Per Week: 35

    General Responsibilities

    The Senior Community Connection (SCC) program at JASA provides brief yet intensive case management services to older adults to ensure they are linked with appropriate long-term services that ensure independent, safe living in the community. The Social Worker at SCC provides comprehensive assessments of social supports, financial needs, housing, legal, health, and mental health capacity and develops a service plan to address identified needs. The Social Worker works collaboratively with SCC case manager, registered nurse, and pastoral counselor to ensure clients access a multidisciplinary team of supports. The Social Worker provides assessments via office visits, virtually, and through home visits and provides information and guidance to clients and collaterals regarding the normal physical, social, and psychological development of aging, challenges to functioning presented by conditions of impairment, disease, social stresses, and dysfunction; and ways of coping and preserving individual functioning and autonomy. This worker will cover the Bronx catchment area. This role currently offers a hybrid work schedule.

    Primary Responsibilities

    • Comprehensive assessment utilizing prescribed standardized assessment tools and a person-centered care approach.
    • The development and implementation of a written service plan for the provision of the full range of social services according to client needs and eligibility for services.
    • Individual counseling utilizing a trauma informed approach and motivational interviewing techniques to address concerns related to aging and accessing services.
    • Eligibility screenings for services and public benefits.
    • Documentation gathering and application submission for public benefits and entitlements.
    • Referral to, and maintain cooperative relationships with, other community agencies to meet the needs of clients
    • Perform outreach and education activities to increase awareness of the program, its core functions, its objectives and value to the community.
    • Participate in professional training to acquire and update skills (i.e. attend in-services).
    • Complete required documentation, case records, reports, and statistics utilizing computers and databases as required.
    • Inform supervisors of situations of crisis or circumstances where the worker requires further guidance.
    • In all responsibilities listed above, utilize a strength-based, person centered approach to support the individual’s independence and self-determination while working with a diverse population and respond to all individuals in a professional, non-judgmental and culturally appropriate manner.
    • Perform other related tasks as requested or assigned.

    Qualifications

    • MSW or related Master’s level degree (e.g. human services, social services, public administration, nursing or public health)
    • Professional experience relevant to working with an older adult population
    • Desire and ability to work collaboratively as part of a committed, integrated team
    • Excellent interpersonal skills, including the ability to communicate effectively and concisely (both in writing and orally)
    • Professional maturity, good judgment and ability to engage with others
    • Knowledge of Spanish is helpful
    • Strong commitment to JASA’s mission
    • Basic computer skills; familiarity with an electronic client record system preferred
    • Ability to travel using mass transit
    • An understanding and appreciation of the roles of race, religion, ethnicity, sexual preference, and individual values as they relate to serving clients
