Community Care Cooperative (C3) is a new, 501(c)(3) not-for-profit Accountable Care Organization (ACO) made up of 17 Federally Qualified Health Centers (FQHCs). Our mission is to leverage the collective strengths of FQHCs to improve the health and wellness of the people we serve. As of January 2019, C3 membership is estimated to be approximately 123,000 MassHealth beneficiaries.
