CCH provides specialized care and support for patients and their families. It is a community based organization committed to providing the highest quality of hospice care and comfort for persons int he final stages of life with a goal of maximizing quality of life. The hospice team includes nurses, physicians, social workers, chaplains, counselors, home health aides, and trained volunteers to help patinets and their families live as fully and as confortably as possible.
