Medical Home Network

Medical Home Network (MHN) is an innovative public-private partnership working to restructure the way care is delivered and financed. MHN has developed MHNConnect, a cutting edge online portal that unites 20 hospitals and over 155 primary care providers in order to enhance care coordination and quality, improve access, and reduce fragmentation and cost, all while reinforcing the medical home.

Recently, MHN formed a value-based, provider-driven Accountable Care Organization, MHN ACO, LLC, which will leverage the experiences and lessons learned across the network over the past 5 years and accelerate movement toward a fully integrated delivery system focused on improving healthcare delivery and the healthcare experience in Chicago’s safety net.

As the cornerstone of the area’s safety net, MHN aims to not only restructure the way health care is delivered and financed for 170,000 current Medicaid beneficiaries covered by CountyCare, but also to rapidly impact new populations. Ultimately, MHN seeks to create a framework to meet the needs of all vulnerable populations.
