The Hudson Valley Care Coalition
(HVCC) is a coalition of providers that seeks to improve consumer access,
engagement and health outcomes through an innovative, integrated model of care linking
social determinants of health with medical and behavioral health services. This innovative, regional collaboration seeks
new opportunities for integrative care using evidenced based and promising
practices to achieve more efficient and better health care for consumers and
families in the Hudson Valley region.
