    Position Title: Case Worker

    Reports to: Social Service Unit Supervisor, Leonard Covello Older Adult Program

    Location: 312 East 109th Street, New York, NY 10029 Status: Full-Time, Monday through Friday (9 am- 5 pm) Compensation: $49,000.00 annual salary

    Position Summary: The Covello Older Adult Program is located in East Harlem and occupies a 28,125 square foot building. It is open to anyone over the age of 60, regardless of where they live. The program provides socialization, recreation and education through a wide array of daily activities including breakfast and lunch, exercise and dance classes, health and wellness activities, arts programs, trips and much more.

    Case Assistance is offered onsite and by phone, to all members/ participants including our satellite center, Lehman Village. Assistance is offered to those who need help with applying for government benefit programs, advocacy related to housing applications and tenant concerns, navigating the health care system, accessing legal support and connection to mental health and other community resources that will improve quality of life.

    Primary Responsibilities:

    • Maintain an ongoing caseload and appropriate contact as per NYC Aging requirements through office visits, phone calls and interactions throughout the program
    • Conduct comprehensive assessments , provide supportive counseling and/or crisis intervention and work with clients, families, caregivers, and other professionals in coordinating the care plan
    • Assess all clients for emergency grant programs and stipendiary grants
    • Maintain compliance with completing and tracking required case assistance and information and referrals units
    • Maintain compliance regarding service provision, deadlines, and documentation in preparation for audits
    • Utilize online platform to maintain detailed client records including case notes, referrals, and client assessments
    • Collaborate with Supervisor to create an organized and accessible way to store client paper files
    • Participate in staff meetings, agency events, in-services trainings and conferences as required
    • Meet with Supervisor once weekly for formal supervision and at other times as indicate
    • Exhibit awareness of agency and community resources and effectively utilize both in the course of working with staff and clients
    • Exhibit ability to incorporate Mission Statement philosophy in work with staff and clients
    • Exhibit ability to work well within agency and engage appropriately with co-workers within the Covello Older Adult Center and other CBN programs
    • Accept other assignments, as appropriate, to meet the needs of the program and individual clients

    Required Qualifications: Bachelor’s degree with relevant experience. The Case Worker should have an affinity for working with older adults, have strong interpersonal skills, and work well with clients and colleagues. They should be good communicators with strong written skills, attention to detail, and a facility with data input. The Case Worker should have the capacity to manage a caseload, meet deadlines and maintain adequate records. Bilingual English/Spanish a must.
