The Patient Navigation Model partners an individual trained to navigate the healthcare system with a patient, to help the patient overcome barriers to timely access to care throughout the healthcare continuum. Timely access to care increases the effectiveness and efficiency of medical care delivery.
We achieve our mission globally by training effective patient navigators, providing consulting and operational patient navigation services, certifying patient navigation programs, and advocacy.
Our mission improves public health and saves lives by narrowing the disconnect between the discovery of medical care and the delivery of medical care.
