    Horizon Health and Wellness (HHW) provides integrated health care that addresses the whole person and promotes wellness, thereby helping you sustain your health and the health of your family.

    HHW has locations in eight communities located in 3 counties (Pinal, Gila, Yuma).

    In addition to traditional primary care and behavioral health programs, HHW offers Medication Assisted Treatment (MAT) for persons struggling with opioid addiction and has crisis teams in Pinal County. Habilitation assistance for persons with developmental disabilities is provided in Maricopa, Santa Cruz, Gila and Cochise Counties.
