Description

La Luz Center is a community based organization in the Springs area of Sonoma Valley, with a mission to support individuals and families of Latino communities in the greater Sonoma Valley attain their dreams and aspirations by providing opportunities and access to knowledge, skill building and resources. We are currently seeking a candidate to join our family resource center, based at a local elementary school. 

The El Verano FRC Coordinator will help meet the mission of the EV-FRC which is to connect families to services and navigate resources to meet El Verano Elementary School students’ needs for academic success. The ideal candidate has experience working with families, creating or implementing programming, and or experience with 1-on-1 case management and resource navigation and is bilingual. This position involves both coordination of EV-FRC programs and working 1-on-1 with clients. Typical schedule will be Monday through Friday, with early evenings and weekend events a few times a month. 

They will be the point of contact for FRC specific services and will work collaboratively with the wider La Luz Center staff, service partners, co-located service providers, El Verano Elementary administration, faculty, and staff. The El Verano FRC coordinator works alongside the EV-FRC Parent Navigator and  under the supervision of the Family Services Manager.

Essential Duties and Responsibilities:

  • Provide coordination of the day-to-day activities of the EV FRC’s services/programs, working with the Family Services Manager to meet set goals and strategic directions
  • Ensure programming goals, as outlined in the FRC Program plan and grant deliverables, are being planned and completed
  • Assists in program evaluation by conducting post workshop evaluations in a timely manner, tracking output and outcome data, and reporting on results
  • Communicating with El Verano FRC Parent Navigator to create and execute engagement and outreach plans for the El Verano School parents to further our parent reach and client base
  • Support in coordinating the “Learning Hour” program in the following areas: connecting with new volunteers, being the point contact for the school referrals, and monitoring the program on an ongoing basis to see if changes or improvements can be made to the program
  • Provide 1-on-1 support and assistance to families to support their wellness needs by performing initial interviews of families to assess strengths and needs, developing a service plan with the family, which includes goals, timeline, assessment, and evaluation
  • Maintain accurate and comprehensive case notes and service records of all client interactions in the organization's data system
  • Collaborate and maintain communication with the El Verano Elementary School administration, faculty, and staff
  • Collaborate and maintain consistent communication with mental health partners and be the point contact for sending out referrals
  • Other functions as assigned
