    We are committed to comfort and provide the best healthcare to individuals who are terminally ill and are placed on our program. We provide care through an interdisciplinary team consisting of HHAs,RN, SWs and Chaplins. Our staff are diverse and multilingual. Our service areas are Brooklyn, Manhattan, Bronx, Queens and Nassau County.Our providers will serve patients in their homes, in Skilled Nursing Facilities and InPatient Units.
